Issuance of duplicate of family booklet
Introduction
In case of theft, loss or deterioration of the family booklet, the request for a duplicate must be made to the registrar that issued the family book.
To transcribe the birth of children and other information (for example: deaths, etc.), the applicant must contact the registrar (s) of civil status where has initially made the registrations or transcripts.
Documents required
- Legalised declaration of loss, theft or damage;
- CNIE of the person concerned;
- Birth certificates of children, if any;
- Copy of lost, stolen or damaged family booklet, if applicable.
Procedure
The request for the duplicate family booklet may be submitted directly by the person concerned to the registry office that has drawn up the initial family record booklet or by ordering (through a legalised proxy given to a person of his choice), for the purposes of filing the application and withrawal of the duplicate of family record booklet.
In the absence of family ties, the request may be sent to the registrar who issued the initial family record book through the consular services of the place of residence.
